For as long as I can remember, I struggled with fatigue in everyday life, listlessness, decreased libido and my loud snoring made it impossible for others in the house to sleep. After a night’s stay in the sleep laboratory and the subsequent evaluation of the data, the diagnosis came: sleep apnoea with breathing pauses of up to 41 seconds and an oxygen saturation level during the night of 85% in some cases. The subsequent treatment for the disease was with a CPAP mask, which I could only wear irregularly throughout the night and which severely restricted me.
Luckily for me, a friend introduced me to Professor Sailer's clinic and his jaw rotation method. From the very beginning, I was pleasantly and open-heartedly welcomed by Professor Sailer's friendly team. I was advised personally and professionally throughout the

Is sleep apnoea really curable?
My pneumatology specialist wasn’t very hopeful of this.
Treatment from the CPAP machine and mask left me unsatisfied and looking for something better. Although I had no further breathing interruptions during the night using the machine, there was no other positive effect. So I kept looking on the Internet for alternatives, and I found Professor Sailer. I went to the clinic with a referral from my general physician and immediately felt at ease and, above all, that my problem was being taken seriously. After the extensive, professional and above all very pleasant discussions and pre-examinations (sleep lab, X-ray etc.), I received my diagnosis:
Severe OSAS, increasing mask intolerance, narrow respiratory tracts, upper and lower jaw insufficiently grown forward, and a limited ability to breathe through the nose.
These were reasons enough for me to have the bimax operation on 27 May 2020 performed by Professor Sailer and his superb team. The operation is now several weeks in the past, my sleep apnoea has gone, it’s easier to breathe and every time I exert myself, I get much more air/oxygen. I am tremendously relieved and couldn’t imagine that the difference would be so huge. The healing process went very well and I am forging ahead in a positive frame of mind.

The right step into a new life
I had an operation according to the Rotation Advancement Method performed by Professor Sailer and his team on 27.01.2020. Since then an ordeal lasting more than 20 years has fortunately come to a happy end. It’s as if this operation has given me a new life. In the late 1990s I was diagnosed with severe sleep apnoea which was treated surgically with a laser operation at the Charité [hospital in Berlin] in 1998. Unfortunately, the success of the treatment didn’t last very long. The same problems (severe breathing interruptions and snoring) reappeared after around a year. As I was still relatively young, I tried to combat the effects of the sleep apnoea with lots of sport and a healthy lifestyle. This didn’t help me much, meaning that after many years of wearing ear plugs, my wife insisted that we sleep in separate bedrooms. I consulted various well-known ENT specialists who told me that in my case there was no chance of a cure, apart from CPAP treatment. So I went again to the sleep laboratory and, as a result, was given a CPAP device. Unfortunately I was not able to tolerate it, and thus I had to console myself with the thought that the obstructive sleep apnoea was a disorder that was uncurable in my case.
This was followed by years of things getting progressively worse (obesity, high blood pressure, depression, heart rhythm disorders, gastrointestinal problems, ever-present tiredness). I did not automatically equate these with the sleep apnoea, so I resigned myself to my fate.
Then, in June 2019, I came across Professor Sailer’s website which claimed to offer a cure for this disorder. During my first examination in Zürich in July 2019, I was received in a friendly and very capable way, and was shown the reason for my disorder. The diameter of my windpipe at this time was somewhat less than 2mm. After the consultation with Prof. Dr. Sailer I realised that my other complaints were also caused by this condition. I then decided to have the operation in Zürich, even though my private health insurance did not agree to cover the costs. As already mentioned, Professor Sailer and his team carried out the procedure on 27.01.2020. After two days as an inpatient in the clinic with excellent care, I moved into a hotel and was soon able to travel home together with my wife. Since then I have lost nearly 20 kg in weight, my blood pressure is normal, I am no longer tired, need an average of only 5 hours sleep a night and have a very high performance capability. I also no longer snore and can recommend the Klinik Prof. Dr. Sailer with great enthusiasm. A new life began for me on 27.01.20. Dear Professor, many heartfelt thanks for this gift.

We hope that everyone is in the best of health and that the Coronavirus has given everyone a wide berth.
A year has passed in a flash and I am glad that I took the decision in favour of an operation in May. My quality of life has seen such a positive change since the intervention. We can enjoy our lives to the full again. The pandemic is also making its own contribution in this sense. We have been forced to decelerate and can now, thanks to a more relaxed time, perceive many things that were previously hidden entirely from view. To enjoy the long walks we take with the dog. To really see nature in all its beauty. To marvel at two rainbows right next to each other in the rain, or to see the awakening of nature once again... To soak up all the things that had been neglected in recent years due to my health issues. Quite wonderful.

I must have gotten infected quite early, outside of school, although the Franco-German school was one of the first to close. My course of the disease was mild and I think I owe that to my enlarged airways. Regaining my health, vitality and lust for life
My problems were:
- Loud, irregular snoring
- Sleep apnoea
- Night sweats
- No restful sleep
- Daytime fatigue
- Headaches
- Easily irritable
- Constantly high blood pressure despite taking beta-blockers
Unfortunately, no doctor could determine the cause of these complaints.
Because of a diving certificate (my hobby is diving), I visited a lung specialist. After a lung volume and oxygen saturation measurement, he prescribed ventilation with overpressure to me (CPAP treatment for short).
In the beginning, everything was fine, until after half a year when I started to develop a strong allergy to the respiratory mask (I tried different masks). The CPAP method was no longer suitable for me.
I looked for alternative methods.....
and discovered the support splint (lower jaw protrusion splint).
The snoring stopped to my satisfaction.
But in the sleep laboratory, it was determined that the airways remained equally narrow (the oxygen saturation was equally poor with and without the protrusion splint).

At first, I was still sceptical about everything that was coming. But already after the first consultation, it was clear to me:

My surgery was scheduled for January 23, 2019, at 7:30 am.
I was already in the surgery gown in the prep room, and I was ready. Then Professor Sailer came, took his time and spoke very reassuringly about how he would operate on me. The contents have slipped my mind, but not his incomparably calming, compassionate and empathic attitude. I am still infinitely grateful to Professor Sailer for this.
Nine months have passed since Professor Sailer operated on me.

- Right after the surgery, the apnoeas have stopped completely
- Sleeping through the night, because I don’t have any apnoeas at night anymore
- My sleep phases have increased from 4 to 6 hours before to 7 to 8
hours
- My blood pressure has normalised, formerly 190/100, now 125/75 (I don’t take any
beta-blocker more)
- No fatigue and sleepiness during the day
- No headaches
- During strength training, my heart rate rose to 150 beats per minute before the surgery, now to
maximum 112
- Climbing stairs is no longer a problem: Seven stories up, and I still breathe normally;
before, I had to take breaks in between.

Rotation Advancement Operation By Professor Sailer:
After years of snoring, sleep disturbance, daytime fatigue, difficulty breathing, depression and a stroke, I decided to consult Professor Sailer.
After I was diagnosed with severe sleep apnoea in May 2017, was not doing well with the CPAP mask and my X-rays showed drastic narrowing of the respiratory passages and a growth disorder of the lower jaw, Professor Sailer advised me to have the operation. Professor Sailer and his team carried out the repositioning of the upper and lower jaws (Rotation Advancement) in October 2018.
I recovered from the operation very well. Although I was very tired, I did not experience any pain at all, but rather severe pressure caused by facial swelling.
However, this got better after about a week and even the bruising went. Since I was only able to open my mouth a tiny bit, my meals consisted of soups, juice and purees for five long weeks. After the wires and stitches had been removed, I was able to chew food again. I then discovered that the muscles needed to be stretched again after having not been used for a long time. To optimise this healing process I sought the help of Ms. Gerda Braun (a qualified physio and craniosacral therapist). Another 4 weeks went by before I could chew soft meat.
Now, four months after the operation, I can say that my life has changed for the better. I no longer have breathing problems, no longer snore, no longer get migraines, am able to play sport again and am much healthier. People around me have noticed that I do a lot more, no longer gasp for air, have lost weight and my face is “narrower”.
In the days after the operation I could not go for a walk, it was exhausting for me to talk and it was difficult to eat and drink. I still have some loss of sensation but this is improving each week.
It is a blessing that Professor Sailer operated on me. My family and I are very grateful that my quality of life is now this high.

Sleep Apnea operation in 2004
I left my position as CEO of a private bank at the end of 2001 and reduced my workload to 50%. I made this decision on the advice of my doctors who had diagnosed me with severe sleep apnoea. I have been to the sleeping laboratory three times, and the assessments were very bad. Over 130 apnoeas (average 33 seconds). A recuperation value of around two and a half hours per night was calculated. Besides reducing my workload, I had a respirator made for me at the Swiss Lungenliga (Lung League) and used it every night for three years.
I heard about Professor Sailer and his success in the treatment of sleep apnoea from a friend. I then decided rather quickly to contact Professor Sailer. Already during my first visit, I was optimistic to do something about my sleep disorders. A second opinion I received from the University Hospital of Zurich convinced me that Professor Sailer would be able to successfully perform this non-standard operation on me. It has to be said that both an orthodontist and the attending doctor at the Lungenliga strongly advised against this operation.
First of all, my four large wisdom teeth had to be removed, which caused considerable pain! Then, the big operation took place at the beginning of October 2004. Well, the following four months weren't easy. But from then on, I didn’t need a respirator anymore and was able to sleep miraculously well again. Today, I can say that I have regained my quality of life and I look to the future with full confidence. I was 52 years old when Professor Sailer operated on me; it was probably the best decision I have ever taken.

His experiences before and after Sleep Apnea surgery
Professor Sailer operated on me 3 months ago and I am still totally impressed with the result. As a child, I was involved in a serious car accident, which left me with deformities, particularly in my face. Later, as an adult, I had 7 operations owing to this (supraorbital side correction; palate seam extension; bimax. jaw relocation, 3 x nose correction, double chin, scar correction, dental implant including orthodontic treatment). All these operations were carried out in Germany by “experts” and none of them were able to deliver a satisfactory result. Another problem was that for 23 years I hadn’t been able to breathe through my nose and neither the HNO doctors nor the surgeons had been able to – or had wanted to – solve the problem. What’s more, none of these “experts” had diagnosed a sleep-apnoea, which from a purely visual perspective, was more than likely to be present.
My jaw was positioned so far back that these “experts” should have noticed it. I obviously wasn’t able explain the symptoms myself. All in all, I had given up and resigned myself to having to live with the problem. Then I came across Dr Sailer and it was above all his philosophy (functionality and aesthetics) that impressed me. He corrected all the mistakes of the other surgeons in a single operation. And I must say that I have never woken up in such as relaxed way after an operation (I even asked the nurse if I had already been operated on – she assured me that the operation had already taken place :)). Later of course, I realised this; it’s not as if you don't notice any difference afterwards. I was so wonderfully looked after – this was something I hadn’t experienced at all in German hospitals. That’s also one of the reasons why I remember the time so well, despite the operation. They took very good care of me, which naturally made things easier. So, and now? Things are going better than they ever have done: the sleep apnoea has gone; I sleep like a baby. I can breathe through my nose without difficulty; I don’t have to take any medication to reduce the swelling to get through the day. And my face looks more aesthetically pleasing too. It was the best decision of my life. It’s as if I had received a new one. I just wish that I had known about it and had it done much, much earlier (I could have spared myself all the previous operations). But, better late than never. A thousand thanks, Herr Professor.
March 2018, surgery after cpap mask complaints

In 2016 I was diagnosed with severe sleep apnoea syndrome after suffering restrictions for many years with night-time blood pressure crises, insomnia, daytime fatigue and finally cardiac arrhythmia. The polysomnography showed severe REM-related OSAS. In view of the marked nature of the results, CPAP therapy was initiated. For over a year, I consistently wore the mask despite numerous problems. These manifested themselves in a wide range of forms: continual adjustment of the placement of the mask during the night (I tried various different models), swelling of the nasal mucous membranes, dry lips and nasal and oral mucous membranes despite the use of a humidifier, frequent conjunctivitis, continued severe fatigue and sleepiness in the daytime, concentration and memory disruptions, high blood pressure despite medication, in particular high values during the night, frequent urination at night, a sharply distended stomach and shortness of breath upon minimal exertion. I found the continual fatigue and my nightmares and states of anxiety to be very troublesome. The anxiety and nightmares prevented me from resting and caused me to go to bed later and later out of fear. In the morning I constantly had the feeling that I had not slept. My mental state got worse and worse, until finally I became depressive. This was a massive and no longer tolerable reduction of my quality of life
The search for a solution led me, via the Internet, to Professor Sailer’s specialist clinic in Zurich.
I went to the clinic on 07.07.2017 and a detailed diagnosis was carried out. I was diagnosed with severe obstructive sleep apnoea syndrome, retraction of the upper and lower jaw (class 2 skeletal dysgnathia) and hypoplasia of both mandibular condyles, whose growth is retarded, and extremely narrow oropharyngeal respiratory tract with a diameter of 8 mm.
As an alternative to the CPAP treatment, corrective osteotomy of the jaws in terms of a bimaxillary rotation advancement of both jaws was proposed, whereby the oropharyngeal respiratory tract is greatly enlarged and the OSAS is treated causally.
I decided to have this operation carried out because, aside from the restrictions mentioned above, the obstructive sleep apnoea is life-threatening and leads to a shorter life expectancy. For example, the latest multi-centre study in the New England Journal of Medicine shows that CPAP treatment does not lead to a longer survival period.
From the outset, I felt that I was in good hands with Professor Sailer and his team. I would particularly like to mention Professor Sailer’s calm, empathic manner when he talked to me and my wife. In a very detailed discussion, which was perfectly clear even for a layman, we were given the medical facts regarding the operation and the course of healing. I had the opportunity to talk to a patient shortly after a successful operation. Due to the positive feedback, I was reaffirmed in my decision.
Although my medical insurance fund (DAK Gesundheit) refused to cover the costs, I decided to undergo the operation on 7 November. 2017 as a self-payer.
The atmosphere was pleasant when I arrived at the clinic on the day of the operation. I would like to stress that Professor Sailer talked to me again at length before the operation took place, which strengthened my confidence in him.
The post-operative care was excellent and I found the entire team to be caring and professional, particularly Dr. Sailer.
Following the operation, I experienced the consequences mentioned in the briefing, such as swelling, restrictions on swallowing and sensitivity disruptions. In the following weeks, I fed myself primarily with high-calorie, protein-rich shakes and pureed food. I did not experience any pain. My condition improved from week to week. It was a very special experience to be able to sleep immediately after the operation without any equipment or masks, without nightmares, and with restful sleep phases. The daytime fatigue has disappeared, and at 72 years of age I once again feel good and have a good quality of life.
The decades of experience of Professor Sailer and the fact that he successfully developed this type of operating technique affirmed me in my decision to have the operation performed in his clinic in Switzerland and not in a clinic in Germany. His method has been successful in all cases so far, as it was in mine.

I suffered from sleep apnoea for many years and have experience with a sleep splint as well as various respiratory masks. However, as the breathing interruptions became more and more serious, I finally decided to have an operation in Zurich with Professor Sailer. I can only advise everyone with similar problems to do the same. The operation is painless and involves staying five days in hospital. A week later the stitches are removed and a month later the wires in the mouth are removed. Only the swelling is unpleasant, but that subsides in as little as a few days after the operation. Although it is difficult to eat in the first few days, I received excellent care in the clinic in this regard. I was fortunate that my private health insurance covered all the costs. Had this not been the case, I would have also financed some of the cost on my own. Sleeping soundly at night again was important to me, as well as minimising any secondary diseases that may have resulted from the breathing interruptions. I would definitely have the operation done by Professor Sailer in Zurich again if necessary, and was highly satisfied with the result.

My family and I have spent one month in Zurich for my father’s surgery of sleep apnea; a maxillomandibular advancement surgery performed by Prof. Sailer and his brilliant team.
Seven months before surgery, my father went for a polysomnography because of a constant asthenia and snoring during sleep, he was diagnosed with severe sleep apnea (Index of 45/hour).
He started CPAP during sleep but could not use the machine regularly, he changed devices several times but in vain; he was simply not tolerating any facial mask. He even tried dental prosthesis and Surgery (septoplasty & uvulectmoy) but nothing helped. His performance declined so much that he took long periods of sick leave, did not do sport anymore and became depressed and constantly anxious
We finally came across the Professor Sailer’s klinik website and booked an appointment for a medical consultation. We were attended and welcomed in a very friendly way, Prof. Sailer gave us all necessary explanations and surgery was scheduled a month later.
My father, to whom doctors said he should wear the mask for his entire life, is now completely freed from sleep apnoea and can finally have a restful sleep.

It all began in 2012 when I woke up tired, although I had slept more than adequately. For months, it was hard for me to work efficiently during the day and find any motivation for sports and other activities.
My doctor checked me thoroughly and referred me to a sleep laboratory. It was established that I was suffering a huge amount of breathing interruptions (apnoea). As a result, a breathing mask was prescribed to wear at night.
It may be acceptable to some people to come to terms with using a sleep mask for the rest of their life, however the mask bothered me enormously during sleep. In the morning when I woke up, the mask was often next to my bed. This was an unacceptable situation for me. I looked for a permanent cure for my sleep apnoea.
During my research on the internet I became aware of the Klinik Professor Sailer.
Having read the information in detail, I decided to take advantage of the free consultation in Zurich. The clinic is located close to the airport and is wonderfully accessible by public transport. The first impression was very positive.
When I entered Klinik Professor Sailer, I was greeted warmly at reception and then received by Professor Sailer's wife, Dr. Sailer. I was expertly informed in a most conducive atmosphere about the spectrum of options available to patients.
After the consultation, I was introduced to Professor Sailer himself. Professor Sailer is a very relaxed, pleasant and warm person. I felt very comfortable during the tests, during which everything was clearly explained to me and my face was analysed. Afterwards, radiographs of my skull were created and Professor Sailer illustrated how my problem would be treated and what results he could achieve.
An individual approach was basically created that would cure my sleep apnoea while taking into account my facial aesthetics, which was very important to me. This prognosis is of course only possible due to his enormous expertise and his unparalleled experience.
For me it was clear that he should perform the operation. I only had to be concerned about the financial part. In many cases, the cost of the operation is covered by insurance, but not in my case.
In 2016 the time had come. I had been studying this subject very intensely over the last three years and it felt almost unreal to be finally taking this step.
I went to Zurich to have the preliminary investigation and met a patient who had already completed the procedure. This dispelled any lingering doubts, because I had been worried that I might perhaps have an artificial appearance after the operation. The Austrian male patient looked very handsome and I would never have suspected that he'd undergone surgery. He recounted his experiences to me which was also reassuring.
The day before the operation, I travelled to Zurich, checked in at the hotel and then went to Klinik Professor Sailer for my teeth and oral cavity to be cleaned. Imprints were taken and I was then introduced to the anaesthesiologist to have any relevant questions about anaesthesia clarified.
The next morning I arrived at the hospital at around 6:30 am and was shown to my single room with private bathroom. I put on the surgical gown and then everything happened very quickly. I was welcomed by Professor Sailer and the other doctors and whose words encouraged me.
I remember being woken up around 7 pm. I coped very well with the anaesthetic, probably due to the gentle anaesthesia given by an experienced anaesthetist.
I could not eat for the first 2 days and lost about 8 kg in weight. The pain was bearable. I was able get up though and take a walk with Ms Jelic in the Klinik garden. On the 4th day I was picked up by my parents. The 6 hour drive went relatively well.
After arriving home, I could eat only freshly squeezed juices and vegetable pulp for the first week. I also took calorie shakes so as not to lose any more weight. My weight had now dropped from 92 kg to 79 kg.
After another 10 days, the wires were removed from my mouth, because my jaw did not have to be fixed together any more. The swelling went down enormously. Every day it was a little bit better.
90 days after the operation, I went for a check-up. Photos and three-dimensional X-ray images were made to see the results (which were becoming more and more evident). The new positioning of the jaw could be clearly seen, causing an ante-face. Today I can breathe better than ever before. I need less sleep and am active throughout the day. No one has mentioned my "new" look because not very much has changed in my appearance. The result can be seen in my profile though. In the coming months, plate material and screws will be removed.
In summary, I can say the following:
My experience with Klinik Professor Sailer has been entirely positive. The positive effects of this surgery on my personal and professional life are immense. Today, performance and appearance play a crucial role. I have been to many doctors and nobody was as expert and experienced as Professor Sailer.
Everything about him is good on a human level, he is a very pleasant, serene person with a lot of empathy.
His wife Dr. Sailer is also a very friendly, skilful person and I was very well looked after and advised by her. The clinic and the processes behind the operation were professional and well structured. The staff, especially Ms Tejic were very nice and looked after me extremely well. I can thoroughly recommend anyone to at least have a free consultation in order to form their own impression. My symptoms and the search for an explanation
About 2 years ago, my partner told me for the first time that as well as my usual snoring, she had also noticed that I had long and frequent breathing stops during the night.Unfortunately I ignored this problem at first. I didn’t know it was a life-threatening problem called sleep apnoea.
My persistent tiredness and also dangerous falling asleep momentarily while driving were serious problems.
I wondered why I had to get up to go to the toilet 4 or 5 times a night and why I was always bathed in sweat, even during winter in a cool bedroom. It was only later that I learned these were all signs of sleep apnoea.
Sleep laboratory, initial medical examinations and disappointing advice
Urged by my partner, I contacted the sleep laboratory of the University Hospital in Erlangen. There they discovered that I was suffering from a severe form of sleep apnoea. According to the measurements taken there, I had an average of 72 breathing stops per hour during the night while sleeping. The doctor treating me informed me that in my case there was no suitable operative procedure at all. The only sensible method of treatment would be to use a so-called CPAP pressure mask for the rest of my life. Fortunately, this information was wrong.Knowing what I now know today, I ask myself why I wasn’t told about the pioneering operation methods at the Sailer Clinic when I was at the sleep laboratory of the Erlangen University Hospital.I find it hard to believe that they did not know about it there. Or was this life-saving information deliberately withheld from me? I don’t want to accuse the doctors at the sleep laboratory of the Erlangen University Hospital of anything, but I am extremely disappointed with the advice I received there.
Incompatible with the CPAP breathing mask
I used the CPAP pressure mask for 30 days. I did not find it any help. I wasn’t able to keep it on for a single complete night. My partner was able to observe how I ripped the mask from my face almost every night in fear of being suffocated or in panic. This seemed to occur when I was dozing, so I was usually unaware of it. And I had side effects such as constant flatulence and stomach cramps after wearing the mask.
Desperate search for other treatment methods
After it became clear that I couldn’t tolerate the CPAP pressure mask, I was in great despair. With 72 breathing stops per hour and no possibility of doing anything about it, my life expectancy was not very long. But fate was kind to me because I then stumbled across an advertisement for the “Sailer Clinic” when researching on the Internet.The website presented an operative procedure which was supposed to cure sleep apnoea permanently. As I remembered the statement of the doctors in Erlangen, that there was no adequate operative procedure for me, I was initially very sceptical. Today I know that everything described on the website of the “Sailer Clinic” (which actually seems too good to be true) is 100% correct.
So there is a cure after all
As I realised that Professor Sailer could actually cure my sleep apnoea, I naturally also researched the alternatives. I quickly discovered that it is definitely only at the “Sailer Clinic” that this special operative technique is carried out. So I made an appointment for an initial consultation. Professor Sailer gave me a great deal of his time, explaining the cause of sleep apnoea and the operative procedure in calm, understandable words. He immediately gained my trust with his pleasant manner, and my final misgivings melted away.
On the same day, I made the appointment for my operation, which turned out to be the best decision of my life.
The operation
When I woke up after the operation, I was somewhat beaten up and tired, and felt “like a boxer after the 12th round”. My face too was fairly swollen.Taking into account that it was an extensive and also a successful sleep apnoea operation, however, this was a comparatively small discomfort which was soon forgotten.
I was surprised to discover that I really didn’t have any pain after the operation. I could feel merely a slight ache, which disappeared after a few days.
My life post-op (2 months)
I sleep better than ever. I sleep 7 hours in one stretch and wake recuperated and rested. This would have been unthinkable before the operation.
My partner
Naturally she is delighted for me. She sees how well I am now and the fact that I no longer snore is very pleasant for us both.
My quality of life has improved enormously. I am fitter in every sense and can now breathe wonderfully. All the symptoms of sleep apnoea have disappeared for ever. My partner also notices that the breathing stops of before have completely disappeared. To put it briefly –– I feel reborn, and will be grateful to Professor Sailer and his team for the rest of my life.

My name is Klaus Gattermann, I am 47 years old, married and have two children. I am self-employed as a project developer in real estates, have 15 employees and live in Germany in the beautiful city of Braunschweig.
Situation before the operation...
Around 15 years ago, my dear wife first pointed out to me that I snored very loudly and irregularly. I took this for granted, but was angry when my wife prodded me during the night with the words, “stop snoring for goodness’ sake”. I increasingly had the feeling that I hadn’t slept at all.
I gradually started going later and later to bed; usually between 2-3am. I noticed however that I coped better during the day when I hadn’t slept so much, than when I had slept for 8 hours. For many years I didn’t sleep through the night, but got up several times in the night to go to the toilet; I was also constantly bathed in sweat.
5 years ago I went for an examination in a sleep laboratory. The findings? Severe sleep apnoea with many hourly breathing stops where my oxygen saturation sank to a life-threatening 45%.
Treatment method: CPAP device, or put another way: wearing an oxygen mask during the night for the rest of my life..
I proceeded to try out all the different kinds of devices and masks because I had no success with any of them. My performance during the day declined perceptibly. I fell asleep all the time: at the office, in meetings, in the car, in the evening in front of the television, actually everywhere; other people didn’t understand what my problem was. My own motivation had declined so much that I didn’t do sport anymore and therefore my pre-existing weight problems worsened. Three years ago I also had a blood pressure reading of 280 to 180 (you’re reading that correctly).
I was prescribed tablets immediately (6 per day), but I would have to take these for the rest of my life too.
It was a vicious circle.
Discovering the clinic, first contact and pre-examination
As all my doctors were only aware of the symptomatic treatment with the CPAP, none of them knew or wanted to know about a cure; I then researched on the internet myself. I came across Professor Dr. Sailer’s website relatively quickly, because it is the only one which discusses this issue comprehensively. All other options (there are only 4, by the way) touch upon the issue, but you only receive the supposedly detailed information as part of a personal consultation, and no results are presented.
After I had found out comprehensive information on Professor Dr. Sailer’s website, I had a long, informative and very friendly conversation with Dr. Erika Schwob. We arranged an appointment for a pre-examination in Zurich in the private clinic which I attended with my wife on 05.06.2014. I was greeted in an extremely friendly way, and then had a highly interesting discussion followed by an intensive examination. I’ve never had such a good feeling at a doctor’s appointment in my whole life.
This professor, who incidentally possesses 8 doctorate titles, is not a “normal” doctor who simply does his job; no, this man feels that it is his calling. On the same day, I decided to have the operation.
Operation and healing process
On Thursday 03.07.2014, I travelled to Zurich with my wife and completed the examinations in the clinic within two hours that afternoon. Afterwards we went to the Sonnenberg Restaurant (lovely view over Zurich) and had a wonderful meal. My last opportunity to chew for around four to six weeks.
On Friday morning at 7.30am my 7 hour operation began. As Germany was playing France in the World Cup Football Championships at 6pm, I asked the anaesthetist for a “light dose” of anaesthetic.<br/>The operation went excellently. The anaesthetist kept his word and I saw the match in its entirety.
After two days I left the clinic and travelled with my wife to Quellenhof in South Tyrol to recuperate. As your face swells up and becomes blue and yellow in colour, and you are rather tired after the operation, a few days of rest are a very good idea. By the way, I had no pain the entire time.
After 10 days, we returned from South Tyrol to Zurich, to have Professor Sailer remove all stitches (pain-free of course). We drove home the same day. After a further 5 days I went back to work full-time. Primarily office work at first, because I still had some mild swelling. I was able to speak on the telephone really well only a few days after the operation. As the jaw cannot have strain put upon it for 4 weeks after the operation, it was secured with elastic bands. As a result, it was only possible to drink liquids and eat pureed food. This was no problem, however; quite the contrary in fact, as you lose quite a lot of weight during this time. I lost 15 kg.
Four weeks after the operation, the Professor removes the jaw restraints (this is pain-free of course).
Now the phase begins where you strengthen the muscles through constant chewing and start to stretch open the mouth.
This phase lasts around 6 weeks and is actually the least pleasant, if anything, because the stretching pulls a little. That was the only thing.
Health results
Physiognomy before the operation:
Weight: 117kg
Oxygen saturation day/night: 82%/45%
Blood pressure: 280/180
Snoring: very severe
Sleeping: no deep sleep
Going to the toilet at night: several times
Tiredness during the day: dozing off multiple times

At the end of 2015 I underwent a Rotation Advancement Operation, because I could hardly sleep anymore due to shortness of breath and nothing provided me with any relief. The cause was that my jawbone had not grown to its full extent, which led to a bottleneck effect in my throat. The operation, which Prof. Sailer suggested, planned to rectify the bottleneck without altering the position of my jaw line or disfiguring my face.
I agreed to the operation despite the risks involved as I could no longer bear the present situation, which also carried a number of risks: my weight had escalated to 100 kg; I was 185cm tall, constantly tired, my blood values were consistently deteriorating, in particular the liver and cholesterol values. I had also suffered 2 cases of thrombosis in the past 3 years. My whole body was in a bad condition.
In the operation my airways was widened from 4mm to approx. 15mm, my respiratory problems immediately became a thing of the past! In my specific case the operation did not prove to be a minor procedure as a long-term inflamed sinus problem and a wisdom tooth also had to be treated. I therefore prepared myself for a long recovery period and took advantage of the opportunity to change my diet. After only 4 months I had lost 20kg and many other symptoms had also disappeared: Constipation, reflux, the chronic inflammation of my sinuses. All my blood values were at a healthy level again for the first time in years. I am full of energy again and it shows. Everyone around me confirms it. I feel 20 years younger and would definitely undergo this operation again. Heartfelt thanks to Professor Sailer and his team!

How do you feel today, just a few months after the operation?
I feel fit, well-rested and motivated. I am enjoying work again.
Now, three months after the op, my jaw is also fully recovered. My muscles just need to get used to chewing again after weeks of rest. I no longer have migraines since the op and I no longer need to take my blood pressure tablets. My general well-being has changed very much for the better, also because I lost eight kilos as a result of the op
How has your sleep behaviour changed following the operation? Snoring, night’s rest, tiredness?
My husband can confirm that my snoring has totally disappeared. Before the operation, I would snore in every position and it often woke me up. This placed considerable strain on my husband and me. Since the op, I’ve been sleeping soundly and wake up in the morning feeling well-rested. The daytime sleepiness that has tormented me for years has disappeared. I have a whole new quality of life.
How would you describe the physiognomic change to your face and how have those around you reacted?
My appearance has drastically improved. My double chin has disappeared. I look younger now. I’ve received many compliments from those around me.
How has your husband reacted to your decision (before/after)?
My husband has fully supported me pre- and post-op. Ever since our first visit to the clinic he has had every confidence in Prof Sailer and his team, as have I. My husband accompanied me to the operation, we stayed together near the clinic for 14 days and then drove home again after the stitches were removed.

What convinced you to undergo surgery?
The conversation at the clinic convinced me, where it was confirmed to me that there was a complete solution to my sleep apnoea problem. I also had the opportunity to speak with patients who had already undergone this op.
How did your husband, friends and/or GP react when you told them you had decided to go through with the sleep apnoea op?
My husband understood the op completely, for the reasons mentioned above. My friends were somewhat sceptical but understood that I no longer wanted to live with a sleep mask. My GP congratulated me on the decision.
Did you have any pain after the operation?
No. Even the removing of the stitches and wires was pain-free.
What troubles did you have after the operation and for how long?
The swelling after the op was quite severe. Eating and drinking was a little difficult for the first few days afterwards, but I soon got used to it.
Four weeks after the op, the swelling has gone down to such an extent that I can go back to work.
Talking was a little strenuous at first. Even now, three months after the op, I still have mild tension in my jaw. But this is gradually decreasing as each day goes by.
When were you able to drive home again?
I was at the clinic for three days and then drove to a holiday let nearby with my husband. To save ourselves another long journey we stayed there until the stitches were removed. Twelve days after the op, we drove home.
Would you undergo this surgery again?
Yes, at any time.
What is the most important piece of advice you would give to patients considering the operation?
Have confidence in Prof Sailer and his team and don’t hesitate to have the operation.
Long-term outcome after 12 years

Why did you decide on this major operation?
Because only this type of surgery provides healing, and an extreme and lasting improvement of the quality of life.
How did you find Prof. Sailer?

What did convince you to have confidence in Prof. Sailer’s approach?
He has a very calm and competent manner and was able to spontaneously and fully answer all my questions, underlining his experience. He gave me the impression that he does exactly what he says. More cannot be expected…
What were your major concerns and worries before the operation?
Whether the result of the operation would last for the rest of my life.
Did your family doctor or other doctors help you in making your decision?
Rather not. For example, my dentist advised against it because the possible complications would be in no good relationship to the expected success of the procedure and because the treatment would be extremely expensive (his remark: “A kidney can already be had for approximately € 60,000.”).
Were you nervous pre-op? Why?
No.
How did you feel when you first woke up, and on day one, two, and three afterwards?
In a daze after the narcosis but basically I felt good physically, but very swollen.
Did you feel pain or discomfort? What kind and how long?
Not really. Removing the stitches was not very comfortable but I did not have lasting pain. The stretching of the jawbone muscles was not entirely pain-free.
How did your sleep change immediately post-op, and over the next few weeks?
It changed immediately and lastingly, and to date is optimal.
How did any other health issues change that you might have had: blood pressure, diabetes, weight loss?
Weight loss from less food intake. I had no other health issues.
Did you notice a difference in your physical and energy level? How soon after the op? In what way? How was it different pre-op?
I have the sensation that I breathe differently, i.e. better. In addition I’m well rested in the morning and fit for the day; that is, tiredness paired with a certain “exhaustion” is over.
How did you feel when you saw yourself in the mirror the first few days?
Rather somewhat disfigured, caused by the swelling.
How did your environment react to your changes look?
My mother was startled; her ”child” was gone. Friends and acquaintances were of the predominant opinion that I had a completely different look -- more striking. For those who did not know me before, I look good.
How did you feel mentally post-op?
No noticeable differences in my capacity to memorize. Concentration is markedly better, especially in the afternoon.
How do you feel now, approximately 3 months after the procedure?
Not quite finished yet -- which will likely change after the removal of the screws etc. in my jawbone. I get accustomed very quickly to super sleep, which was the purpose of the procedure. For the rest I have the necessary patience.
How is your physical comfort changed relative to snoring, the quality of your sleep, and your daily activities?
The difference is remarkable. I no longer snore, feel top-fit, well rested and high-performing throughout the day, as opposed to just the early morning hours in the past. I feel energized for activities and as if moved backwards in time.
How did you feel taken care of upon arrival in Switzerland, during your stay and after the surgery. What was your impression of the clinical care?
Super.
What is the most important advice you would give to other patients pre-operatively?
You need patience and discipline after the operation.
What do you think the Clinic Prof. Sailer and its staff could do to improve your experience and results?
I do not have an answer for that. Experiences and results are amply available and the reason for me to have the operation done.
With your current knowledge, would you decide to have this operation again?
Yes, by all means, the change in quality of life is fantastic.

The question how I am now three years after the operation in January 2011 can be answered quite simply. Excellent and terrific as never before, reborn.
Let us consider the individual aspects more closely so the rationale for Prof. Sailer’s Rotation Advancement can be fully understood and there is nothing to be afraid of.
Sleep: Finally; I sleep well. 5-6 hours without interruption is now no longer a rarity but rather the rule. In the morning I'm fit and well rested; no more night sweats; my performance has increased noticeably at all levels. I am no longer anxious about nodding off while driving. In the evening I feel a natural fatigue which makes me sleep well.
Bronchitis which has tortured me regularly sometimes up to 4 months at a time has disappeared. No more annoying sputum in the morning.
Sports: I am regularly on the road by bicycle and actively do spinning. In this high-performance sport I need a lot of air and now I get enough. Leg cramps are no longer an issue.
Blood Pressure: I can now live without blood pressure medication. Regular checks usually show the upper value at 120-140 and a range of 65-85 for the lower value. It is remarkable that I no longer have nose bleeding since being operated on.
Tragic: My health insurance would not cover the cost of the operation. They would rather pay for the cost of a heart attack than cover this operation as a preventive measure, which would be 5 times cheaper.
Snoring? This famous tip of the iceberg of all the nuisances associated with apnea is no longer an issue.

Dear Mr Urs Fäh, You have undergone the operation and I would like to ask you some questions about it. I have been suffering from sleep apnoea with all its side effects for several years. As I don’t find using the sleep mask very helpful, I would also like to have the operation.

First of all I would like to warmly congratulate you on your decision. I would be delighted to answer your questions. After I read in forums about those suffering on using a mask, the decision was clear to me. I could do without the discrimination of having to grapple with a mask at night.
About 2 years ago (end of June 2012), Professor Sailer corrected the problem which had been an impediment since my birth. As soon as I woke up from the anaesthetic, I was able to breathe freely for the first time in my life; as though I was reborn. I did not have any pain and therefore I voluntarily refused the pain relief medication offered to me. After 5 days I returned home and a week later I had already gone on a 3 hour hike, without any problems. Around 4 weeks after the operation I started to eat solid foods again. Prior to this, my wife had pampered me by simply putting my normal meals through a blender and I was therefore able to take the “meal” like soup with the help of a straw and I didn’t have to forgo delicious food.
I did not have high blood pressure, despite 20 breathing stops of up to one minute per hour (sleep laboratory), as I had benefitted from low blood pressure from my childhood onwards, despite having asthma. Sleep problems caused by apnoea became a thing of the past from the moment I woke up from the anaesthetic. The apnoea is definitively gone. When I wake up in the morning, I want only one thing: to get out of bed and experience the day. After the operation my ability to work under pressure was far superior to previously (despite the fact that I am 64 years old).
The reactions of other people were actually more or less obvious to me, because they came up to me very openly and spontaneously and could hardly believe the facts about the operation.
Of course my appearance had changed a little, but this tended to be a positive thing (according to the reactions of other people).
The only negative thing would have been if I hadn’t have decided to have the operation. But from the very beginning I had trust in Professor Sailer. He is easily the most eminent authority in this field. He also dedicates himself selflessly to the “Cleft-Children” organisation, which he himself founded.
I wish you the same “rebirth” that I have experienced.

Have you had sleep apnea treatments before you decided for the "Rotation Advancement Operation"?
I used a CPAP device 12 years long.
Did this treatment help you to reduce your symptoms?
This mask caused loud noise and that gave me and my wife unquiet nights. In addition, the face mask had to fit tightly, so I had every morning significant pressure points on the face, which disappeared only very slowly, sometimes up to four hours after getting up. Still today I have at some old bruises small visible broken capillaries. I am also convinced that my shoulder pain, was caused by escaping air of the pressure device, because now, as I am no longer wearing a mask it has disappeared completely.
What was your impression after the first consultation with Prof. Sailer?
Right away confidence in him and his abilities.
How did you feel when you woke up and the day after the surgery?
A bit uncomfortable, however, I had no pain.
After a few weeks and months; what differences did you notice in terms of sleep quality, activity, daytime tiredness, high blood pressure, body weight or other symptoms?
Before, now and then I felt lack of concentration and my wife was worried that I become forgetful.
This has now improved considerably. I feel no more daytime sleepiness. Now I can make sports without feeling beathless. Furthermore I feel a high physical vitality.

Very good.
How long did you had to stay for the treatment?
The surgery was on Wednesday. On Saturday I came home and in the afternoon I went for a walk with my dog. The day after we were out in the forest for 2 hours.

How do you feel now?
I feel very well.
Immediately after the operation?
Immediately after the operation you stay in hospital for a short time and are very well looked after. I never had any pain.
Healing phase?
That went very well for me. Everything healed well. I drank liquids through a straw and I just cooked my food very well, so that I could “mush” it with a fork.
How long did it take until the swelling disappeared?
Well, I went back to work after 4 weeks and you could hardly see anything.
How long did it take for the end result to be visible? How is your appearance? Different from before?
Everything was visible very quickly as I also had my nose corrected at the same time. This didn’t have anything to do with the apnoea operation though.
How long ago did you have the operation?
I had the operation on 22.12.2011 and I am feeling very well. No breathing stops anymore; I feel much fitter and have also stopped smoking.
How is eating for you – do you have any pain?
When I was able to eat anything I wanted again around 6 weeks after the operation, I did have an uneasy feeling at the beginning.
I found it very difficult to bite fully again at first. But it was more the fear that something might happen. Professor Sailer had even given me the “command” to really bite properly again.
Dear Ms W., if you need to have the operation then I can assure you that you are in the right hands. Professor Sailer is not only a great doctor but he is also a great person, too. I felt in very good hands during every check-up. The only unpleasant thing is when Professor Sailer removes the wires. But it is soon over.

How did you find out that you suffered from sleep apnoea?
I never felt refreshed in the morning after a night’s sleep. During all sporting activities such as jogging, swimming, yoga etc. I had to breathe very loudly and deeply and felt increasingly out of breath.
How did you find out about Professor Sailer?
Via the supplement to my Sunday newspaper.
Had you already tried treatment for sleep apnoea before you decided on the rotation advancement operation?
I had used a CPAP device for 12 years.
Did this treatment help to reduce your symptoms?
The mask made a loud noise which did not allow me or my wife to have restful sleep. I also had to fix the mask to my face and therefore I sometimes had visible pressure marks for up to four hours after getting up, which disappeared only very slowly after massaging them or taking hot and cold showers. Even today I have small visible areas of burst blood vessels at the site of some former pressure points. I am convinced that my shoulder pain also stemmed from the leaking air of the over-pressure machine because now that I no longer wear the mask, this has completely disappeared.
What was your impression after the first consultation with Professor Sailer?
Immediate trust in Professor Dr. Sailer.
How did you feel on the day after the operation?
Somewhat uncomfortable. Freshly operated upon. However I did not have any pain.
After a few weeks and months: what differences did you notice regarding your sleep quality, activity, tiredness during the day, high blood pressure, body weight or other problems?
Previously, I sometimes used to have problems concentrating and my wife noticed that I was becoming more and more forgetful. This has now improved considerably. I do not feel tired during the day and breathe normally without effort during all sporting activities. I also feel a sense of increased physical vitality.
Did you feel that Professor Sailer and his team cared for you well and in a professional way?
Yes, very well in fact.
How long did you have to stay in Zurich for the treatment?
The operation took place on a Wednesday. On the Saturday after that I was already at home and went for a walk in the afternoon with my wife and dog. The next day we walked through the forest for a whole 2 hours; I was in regular contact with Professor Sailer just to be on the safe side.

Because only an operation brings about a cure and improves your quality of life significantly and permanently.

Did your GP or other doctors help you with your decision?
Not really, because my dentist for example advised me that the possible complications would be out of proportion to the success and that the procedure would be extremely expensive (his remark, “you can even get a kidney for around €60,000”). He did admit, however, that he was unable to appreciate the degree of suffering caused by sleep apnoea because he didn’t have any experience of it. The pneumologist was very satisfied with the results of the treatment using the CPAP mask after my check-up with him. He informed me that there could be alternatives to the CPAP device in the not too distant future.
Were you nervous before the op? And if so, why?
No.
How did you feel when you woke up after the operation, and the next day, and on days two and three?
Dazed after the anaesthetic, otherwise in good physical shape but very swollen, however.
Did you have any pain or discomfort? If so, what kind and for how long?
Actually no, the removal of the stitches wasn’t very pleasant but there was no persistent pain. Stretching out the jaw musculature was not completely pain free.
How has your sleep behaviour changed following the operation? Immediately afterwards and during the ensuing weeks?
Immediately after and ongoing, at the moment - perfect
How have other health problems changed? Blood pressure, diabetes, weight loss?
Weight loss = because of less food. Apart from that, I didn’t have any problems.
Have you noticed a difference in your physical fitness and performance level? How soon after the op? In what way? What was it like before the op?
I personally feel that I breathe differently, i.e. better. In addition, in the mornings I am well rested and fit for the day; in other words, any tiredness during the day and my previous level of exhaustion have disappeared.
What did you think when you looked at yourself in the mirror in the first few days after the op?
My face was a little distorted, which was due to the swelling.
How did those around you react to your changed appearance?
At first my mother was horrified that “her child” had gone. The prevailing opinion of friends and acquaintances: that I look completely different ... more striking. For people who didn’t know me before, I look attractive.
How do you feel mentally after the op? Your concentration, your memory retention?
No perceivable differences in my memory retention. My concentration is significantly better, particularly in the afternoon.
How do you feel now, around 3 months after the op?
Not yet completely finished, which will certainly be remedied after the jaw screws have been removed. You very quickly get used to the fact that you sleep fantastically, which was the purpose of the op. For the rest, I have the necessary patience.
How has your sense of physical well-being changed in relation to your snoring, sleep quality, activity during the day?
The change is impressive. I don’t snore any more, I feel extremely fit, well-rested and productive throughout the entire day and not just during the early morning, as was the case previously. I’ve got energy to do things and feel as if I am living back in my younger days.
Immediately after your arrival in Switzerland and during your stay did the clinic take care of you?
Yes, very well.
What is the most important piece of advice you would give to patients in a pre-operative condition?
You need patience and discipline after a sleep apnoea operation.
In your opinion, what could the Klinik Prof. Sailer and his employees do to improve your experience and results?
There’s no answer to that. The experience and the results are so excellent, and that is what persuaded me to have the operation.
Would you undergo this surgery again?
Yes, absolutely, because the improvement in terms of quality of life is huge.

Unbelievable. I had the operation on 12th January. I’ve been home since 17th January and my recovery is proceeding according to my expectations and I am mostly free of pain. The positive effects of the sleep apnoea operation were soon noticeable. The operation really is a medical wonder. Carried out to perfection by Professor Sailer. After months of decision-making and consultations I decided to have this jaw operation. Tiredness during the day, poor sleep, and my snoring were the crucial factors for me at first. After all the examinations I then discovered I also had a lack of oxygen in my blood, too many breathing stops per hour and a risk of stroke and heart attack, making the operation essential. I can’t thank Professor Sailer enough. His infectious geniality is paired with an incredibly high level of expertise, and great creative and motivational energy. I always felt very well looked after, both by him and his super team.

10 weeks after the sleep apnoea surgical procedure, Mr H. Müller wrote
10 weeks have now passed since the op. I have had a great recovery and recuperated very well. I have been working normally for 6 weeks. My sleep quality has improved enormously: no restless sleep, no waking up dripping in sweat during the middle of the night. In the morning I feel awake and rested; I’m no longer someone who lies in. Even without a midday snooze, I am no longer tired during the day. Long trips on the motorway, which I need to make for my job, are no longer so tiring, because I don’t need to be scared of falling momentarily asleep.. And the most important thing: my blood pressure has also lowered without medication. My blood pressure, with a systolic value between 118 – 135 and a diastolic value between 66 – 84, lies in the target range. My resting heart rate of 45 – 55 is as low as an athlete’s. And I almost forgot: my snoring is no longer an issue. Overall I feel much better.

The question of how I feel today, 3 years after the operation in January 2011, is very easy to answer. Fantastic and better than ever; as if I had been reborn. If we consider the individual points more closely, then the principles of Professor Sailer’s rotation advancement technique can be understood completely and there is no reason to worry.
Sleep: finally sleep well, 5-6 hours without interruption is no longer a rarity but tends to be the rule. In the morning I’m fit and well rested - no more night sweats, and my performance has increased in all areas. No more fear of falling momentarily asleep when driving. In the evenings I feel a natural tiredness which allows me to sleep well.
Bronchitis: which I sometimes suffered from for up to 4 months, has not returned. I don’t cough up annoying phlegm in the mornings any more.
Sport: I regularly travel by bike and do spin-cycling. For this performance sport you need a great deal of breath and I now get enough. Leg cramps are no longer an issue.
Blood pressure: I don’t need to take medication to reduce my blood pressure. Regular check-ups usually show a systolic value of 120 - 140 and a diastolic value of 65 - 85. Interestingly, since the operation I have not had any nosebleeds.
Unfortunately: Health insurance company is reluctant to cover the costs. They would rather pay the costs of a heart attack than for a prevention therapy which is 5 times cheaper.
Snoring? The well-known tip of the ailment iceberg is no longer an issue.

I had to sleep in the kitchen
My wife and daughter banned me to sleep in the kitchen because my snoring was so loud. At first I would sleep in the living room, but the bedroom was next door and I simply snored too loudly. I used to sleep in the kitchen on a mattress on the floor. I felt totally exhausted and without energy every day, as if I hadn’t slept at all. In the evenings I mostly fell asleep in front of the television. Then my wife would wake me to bring me into the kitchen. At least she and my 16-years old daughter could then sleep.
My son thought I was an alien.
When I wore the mask for the first time, it was a stormy summer night. There was thunder and lightening; then the door opened and my young son stood looking very scared in the doorway. I sat down beside him and wanted to reassure him that it was only a storm, nothing could happen. When he saw me, he started to scream in fright, tears rolling down his cheeks. I ripped the breathing mask from my head and took him in my arms. Sobbing, he stuttered: An alien, I thought you were an alien...daddy, why do you have to wear this funny thing? From then on, my son would look fearful whenever I came home.
Called the ambulance because of the mask.
I was on one of my frequent business trips. I always took the portable CPAP device with me since my sleep apnoea had been diagnosed 3 years previous. I had an early meeting the next morning and ordered an early morning call to wake me at 6am. I didn’t hear the telephone. After several attempts to wake me up and out of fear that something had happened, the concierge and an employee opened the door to my room. Both had the shock of their lives when they saw me, and called other employees and the emergency services. Since then, I don’t take the device with me when I travel any more, or I try to avoid any kind of travelling.
Why so tired, you’re not putting any effort in
I am so tired all the time, I feel like falling over. I fall asleep at any opportunity like the theatre or cinema trips, or even just watching the news, despite trying very hard not to. My wife didn’t have much sympathy for me. It was very hurtful when she used to say: Why are you so tired, you’re not putting any effort in. Now I wear a CPAP device. The problem is that I toss and turn a lot when I sleep and have to put the device back on my face again. Nearly every morning, I wake up to find the CPAP is off, because I take it off without realising. My doctor told me that there was no other treatment.
No sex for years
I am 47 years old and have been married for 20 years. My wife and I used to do sport together often. We have a business together. I put on 30 kilos in weight, have been snoring for years and have breathing stops. My wife has now filed for divorce after many years of problems. The main reason is that she herself is often at her wit’s end, also because I have become so unattractive and we have not had a sexual relationship for years. My daughter said I was a “lazy pig” because I can hardly work. I am completely depressed and now wear various CPAP devices. The placement doesn’t work well for me because I have a very small chin and therefore have a beard; the nose device gives me nosebleeds. I have now heard that it is possible to cure this with a jaw operation. My only comfort at the moment is that I have got myself a dog which forces me to go for a walk

When I returned home in 2012 from my stay in the sleep laboratory with the new CPAP device, my wife and I were able to enjoy life again. Peaceful sleep with sufficient oxygen – in the morning I was rested and refreshed and ready to take on the day.
It took just 6 months however,
for old and new problems to arise little by little. Increasing colds, coughs and sneezes – recently, my nose was continuously blocked up for half a year. Because of this, it was very difficult for me to breath through the mask. I visited the ENT specialist again, but unfortunately, no new approaches were available. For him, the CPAP method was the “gold standard” and that's that – so what did I want then?.
After a few hours of Internet research about the issue, I found some independent information. Various self-help groups are speaking out in their respective forums – I also always had the impression that there was no alternative to CPAP.
But then I came across Schlafapnoe.com. This is a website that Professor Dr. Sailer from Zurich operates. It was reported here for the first time anywhere that OSAS is curable. In the meantime, my second session in the sleep laboratory made me aware of how serious my circumstances were. Immediately action was necessary.
An appointment in Zurich with Professor Dr. Sailer was quickly arranged and took place. In a sympathetic, competent and quiet manner, the professor described to me what, in my case, specifically had to be done. This was only possible on the day of the examination, because his well-equipped clinic could quickly run the necessary tests. A number of previously treated patients were mentioned to me, so I could ask for their opinion. Everything that I saw and heard gave me confidence and assured me that this was the solution to my health problems.
However, after a preliminary enquiry, my private health insurance categorically refused to assume the costs. I was told to consult my ENT doctor and ask for his point of view. Another ENT examination with a young, well-informed specialist gave me however a different perspective. This specialist also confirmed the treatment proposals by Professor Dr. Sailer for my particular case. I then organised private funding for this procedure and planned to take legal action against my private health insurance at a later date. On the 21.1.2016, the procedure was carried out at the Sailer Clinic. An elaborate operation, in which the upper and lower jaw together were separated from the skull and re-attached after a forward rotation. This took several hours under general anaesthetic and was carried out by a talented team headed by Professor Dr. Dr. Sailer. At approximately 5 pm, my wife visited me as soon as I had woken up from the anaesthesia. After one hour, I was able to converse a little bit. After 2 more days, which I spent in hospital, my condition had improved so much that I was able to spent the next 8 days in a nearby rented apartment being cared for by my wife under the supervision of the clinic in order to alleviate the consequences of the operation, such as, in particular, the swelling. On the 10th day after the operation, the stitches were removed in the clinic and the medication and behaviour rules were set for the next three weeks.
I spent those three weeks with my wife in Palma de Mallorca. By the end of this period, long walks, lots of rest and a controlled diet had restored me to a good state of health, which is necessary in order to be able to return to work. On the way home, we stopped at the Sailer Clinic for several hours in order to remove some wires.
I’ve now been back at work for 3 days. My performance has increased significantly because my brain is now well supplied with oxygen and it is now much easier to apply my mind to my work than before.
My wife is also delighted that I now can have a good night’s sleep because she, in turn, is also able to sleep better. To sum up, it is important to say that this operation at the Sailer Clinic has changed my life in a positive way. Therefore, my wife and I would like to wholeheartedly thank Professor Dr. Sailer, his dear wife and his talented staff.

Almost 1 ½ years after the operation, I can say that not only has it cured the sleep apnoea, but it has also changed my whole life.
I can finally sleep well all night long without a mask and without snoring. My partner is sleeping with me again after we had been sleeping in separate beds because of my snoring. I have lost a considerable amount of weight, can finally do exercises without breathlessness and am active all day without symptoms of fatigue.
Looking back:
My partner could no longer put up with my increasingly loud snoring, moved to the other bedroom and made me an appointment with the family doctor to look into my nocturnal apnoea and snoring as well as my constant fatigue. I used to fall asleep all the time, not only while watching TV in the evening or in front of the computer during the day but even while driving. During a trip home from Basel to Aarau, I had dozed off briefly when my tires suddenly hit the curb and I was only able to yank the steering wheel around at the last second, fortunately causing only a fender bender on my own car. I can’t imagine what might have happened if a vehicle would have been coming towards me!
To treat the snoring problem, my family doctor recommended wearing pyjamas with tennis balls sewn in, but after hearing of my constant fatigue he referred me to a pulmonologist. He sent me back home with a “home sleep lab” and when the results were evaluated the next day, the doctor said it was necessary to take immediate action.
36 breathing pauses per hour were measured, with some of them lasting 2 minutes. As treatment, he mentioned minor throat surgery but indicated that this would not be helpful in my case. He referred me to the “lung association”, where I was given a breathing mask that I was supposed to wear for the rest of my life while sleeping. I first tried the biggest mask; however, it gave me feelings of panic and claustrophobia. I exchanged it for various smaller models. The masks and the tube constantly slipped during sleep. I felt totally unrefreshed and exhausted in the morning because I was busy all night trying to readjust everything. These devices were unbearable for me and I was unable to get an uninterrupted night’s sleep. On top of it all, the air to be inhaled seemed to get increasingly “thicker”, as if I were breathing against the air flow.
Meanwhile, I searched the web for solutions to my problem and came across Professor Sailer’s website. At the request of my lung specialist, he told me that although the medical treatment described on the website was the “solution” for me, it was associated with considerable expenditure. I wanted to try it out, especially as I had no other options apart from the unbearable masks, and so I booked a speedy appointment at Professor Sailer’s clinic.
I was quite anxious and then surprised by the professor’s warm welcome and how seriously and comprehensively they looked into my problem. I sent my health insurance advisor the documents from my family doctor, the mask intolerance confirmation from the lung association, which documented that I had tried out all sorts of breathing devices, the result of the sleep laboratory from the lung specialist and the medical certificate with the X-ray images from Professor Sailer. The operation:
About 1 ½ months later, I underwent surgery.
When I woke up in my hospital room following surgery, it first felt as if I had been to the dentist. I didn’t have any particular discomforts. Over the next 2 days, however, I was given painkillers to suppress the unpleasant sensation of swelling and the feeling of strain due to my jaw having been moved forward. The day before my discharge, I was already feeling significantly better. From then on, things constantly improved. I felt better from one week to the next and after about 4 months I was able to eat tender meat again; after 6 months I was enjoying my customary foods again. One positive side effect is that I lost 15 kilos soon after surgery. I used to weigh 114kg and now only 86kg. In order to keep up the sense of achievement, I have adapted my food intake to my new life situation; I now work out, am happy to do without elevators and can walk up to my new office on the 3rd floor without breathlessness. My previous employer had dismissed me due to my constant fatigue and lack of concentration. I am once again working in a bank; following departmental reorganisation, only two employees were kept on. I am one of them thanks to my outstanding performance. That would not have been possible in the condition I was in only two years ago. Six months ago, during my farewell aperitif with my former colleagues, I drank a little alcohol again for the first time after the operation and my partner assured me there was no more snoring at night. All my former colleagues have also confirmed my positive new appearance, and not only because of my weight loss. I myself also prefer the way I look now.
Looking back, I can say that the unpleasant time after surgery seemed very short and that I already felt a real zest for life after 3 to 4 months. There’s no comparison to the way things were before. I have not only gotten rid of the sleep apnoea, the fatigue and their accompanying symptoms, but also the unpleasant idea of have to wear the mask for the rest of my life and the fears of suffering a stroke and having to live with its consequences.
